INITIAL LICENSURE & RELICENSURE

This page is designed to assist our EMS providers and their Agency Coordinators with provider relicensure.

 

NEW FROM IDPH: LICENSE LOOK UP

IDPH-EMS and Highway Safety has rolled out to its website a new online EMS license verification process that can be used to verify individual and ambulance EMS licenses’. The online EMS license verification link can be accessed by the public, businesses and other EMS entities by going to https://dph.illinois.gov/topics-services/emergency-preparedness-response/ems.html and clicking on “this site” under the heading of EMS Licensing and Renewals. Once on the EMS System Licensing page, the individual can start their search by selecting “Search for Licensees” under the a new heading called EMS Licenses Verification.

 

NEW IDPH WEBSITE AND RESOURCES

IDPH now has a new website: https://dph.illinois.gov/topics-services/emergency-preparedness-response/ems

from there you can chose the Forms & Publications tab to print inspection and renewal forms

or the Licensing & Certification tab to renew licenses and vehicles online or look-up a license status.

 

 

INITIAL LICENSURE:

If you took the Illinois State Licensing Exam through CTS/PSI remember you still have to pay for your State of Illinois EMS license before it will be mailed to you. The LICENSURE FEES are listed below and are based on the level of licensure you seek. Once you pass the exam, you will receive a letter in the mail with a PIN # telling you to log into IDPH to pay your fee so your license will print. That website is https://dph.illinois.gov/topics-services/emergency-preparedness-response/ems.html

 

If you took the National Registry Exam and want an Illinois license, you must wait until you receive your NREMT card in the mail to apply for an IL license. Once you have your NREMT card and pass letter bring it to the EMS office where you took your class with the following...

1. NATIONAL REGISTRY TO ILLINOIS LICENSE APPLICATION form

2. IDPH Renewal Notice-PDF Form printable from https://dph.illinois.gov/topics-services/emergency-preparedness-response/ems.html

3. Copy of NREMT card and pass result letter

4. Copy of current CPR Card

5. Money order or cashiers check payable to IDPH for the amount listed below. The LICENSURE FEES listed below are based on the level of licensure you are applying for.

  There are a some key factors to remember when it comes to relicensure:

  1. Make sure the Silver Cross EMS EMS System office has your current address at all times.

  2. Always keep copies for your own records. (As it is ultimately your responsibility.)

  3. Never wait till the last minute. Check your CE every 3 months and track your hours.

  4. Always obtain details of your classes, such as topics covered, site codes and signatures.

  5. Use the FORMS below to help guide you through the process.

 

The State of Illinois has relicensing fees effective with all September 2012 renewals,

as well as initial licensing fees and a fee to request a duplicate copy of your license.

The process for submitting payments is listed below.

Starting September 2012 individuals who apply for initial licensure or for renewal are charged an EMS licensing fee. IDPH issues renewal notices for applicants renewing in 60-90 days and will provide the individual with a PIN number and the web address of the EMS website where he/she will find the on-line licensing and renewal link. If you do NOT receive this notice, email your EMS System so they can reprint it for you. Include your current address in the email.

Upon renewing online with IDPH you will be asked to enter OUR EMS System Number code of 0710.

This link also directs the individual to notify their EMS System Coordinator or Trauma Nurse Coordinator that he/she is applying for renewal. If Silver Cross is NOT your primary System, click here for a list of EVERY SYSTEM NUMBER 4-DIGIT CODE IN THE STATE. One of the drop down boxes on the IDPH renewal website asks for your TNS training hospital. TNS stands for Trauma Nurse Specialist, so if you are NOT a Trauma Nurse leave the TNS Training Hospital blank. Trauma Nurses will know which hospital they were trained by.

 

Another change on the renewal notice is a felony conviction statement. EMT's who report a felony conviction will not be able to complete their application on-line but will instead need to contact IDPH.

 

Click here to view the IDPH MEMO and some on-line SCREENSHOTS of the ONLINE process. This will help the EMS System Coordinators, TNS Coordinators, and EMS providers know what to expect when paying initial license fees, renewal fees, late fees, and/or updating address information.

 

Individuals who receive either initial notification of having passed a licensure exam or have received a license renewal notice will be able to access their records by using their confirmation number (PIN) AND the last four digits of their social security number to verify address, child support status, felony conviction statement and then be directed to an on-line fee payment page for payment via credit card. When the transaction is approved the individual will be provided with a confirmation page, which they may print for their records.

 

Persons without a debit/credit card or who wish to renew through regular procedures may complete their renewal forms and return them to IDPH, Division of EMS, Attention Licensing via regular mail with the fee enclosed. IDPH will ONLY accept an organization's/institution's check, certified check, cashier's check, bank money order, or postal money order. PERSONAL CHECKS WILL NOT BE ACCEPTED and will be returned along with the renewal application. Understand though that mailing the payment takes much longer to process and you may not receive your new license in time.

EMS System Coordinators may request a waiver of the licensing fee for individual EMT's and First Responders who serve exclusively as a volunteer for units of local government or not-for-profit organizations that serve a service area with a population base of less than 5,000 by providing a license fee waiver application form.

 

The on-line fee payment system allows for vehicle service provider agencies to utilize a credit card to pay for vehicle and ambulance provider license renewal fees. Vehicle service providers will be able to log onto the website https://dph.illinois.gov/content/dam/soi/en/web/idph/files/forms/publicationsopremscriticalcaretransportapplication.pdf (click pay vehicle fees in the upper right) and select the vehicle(s) that are due for renewal and pay the fee(s) for the vehicle(s) needing renewal. Vehicle inspections will still have to be completed and entered into the EMS Licensing System for the renewal of the license(s) to be complete and print. NEW or REPLACEMENT vehicles cannot be paid via the on-line system.
